When the GAD-2 is combined with the PHQ-2 we get the PHQ-4.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Details<\/h4>\n\n\n\n<p>It consists of just two items from the GAD-7: &#8220;Feeling nervous, anxious or on edge&#8221; and &#8220;Not being able to stop or control worrying&#8221;.  Like the other measures in the PHQ\/GAD family the stem is \u201cOver the last 2 weeks, how often have you been bothered by the following problems?\u201d and the scoring is on a four point scale: \u201cNot at all\u201d, \u201cSeveral days\u201d, \u201cMore than half the days\u201d and \u201cNearly every day\u201d scored 0, 1, 2 and 3 so the score range is from zero to six.
