It appears at the end of that paper and that appears to be downloadable from https://onlinelibrary.wiley.com/doi/pdf/10.1046/j.1365-2788.2003.00457.x so it is probably legally copyright to Blackwell publishers. Unlike it’s sibling, the Glasgow Depression Scale, it does not appear to have a carer rating complement.
