“GAD” stands for “Generalised Anxiety Disorder” and this is the two item short form from the GAD-7 and sibling to the PHQ-2 (“Personal Health Questionnaire-2”, q.v.). When the GAD-2 is combined with the PHQ-2 we get the PHQ-4.

It consists of just two items from the GAD-7: “Feeling nervous, anxious or on edge” and “Not being able to stop or control worrying”. Like the other measures in the PHQ/GAD family the stem is “Over the last 2 weeks, how often have you been bothered by the following problems?” and the scoring is on a four point scale: “Not at all”, “Several days”, “More than half the days” and “Nearly every day” scored 0, 1, 2 and 3 so the score range is from zero to six. It is described as a screening measure but as with any such very short self-report measures, its screening parameters, even in use where depression is very prevalent, will be terrible and clearly the score range is very limited for any use.
